Information in an inquiry
The contact form prepares an email using the business details you enter, including your name, reply email, phone, pharmacy name and requested service. It opens your device’s email application; you decide whether to send the message.
Sensitive information
Do not submit patient records, prescription details, Social Security numbers, dates of birth, bank records or other sensitive information through this website.
Email handling
The website has no server-side inquiry form and does not store form submissions. If you choose to send the prepared email, your email provider transmits it and Element Licensing receives it at license.element@gmail.com. Email providers may retain messages under their own policies.
